Instructions:
The Heritage Assessment Tool is a set of questions can be used to investigate a given patient’s or your own ethnic, cultural, and religious heritage. It can help you to perform a heritage assessment to determine how deeply a given person identifies with a particular tradition. It is most useful in setting the stage for understanding a person’s HEALTH traditions. The greater the number of positive responses, the greater the person’s identification with a traditional heritage. The one exception to positive answers is the question about family name change. This question may be answered negatively.
The learning activity and corresponding assignment in this topic requires students to perform a heritage assessment with families selected by the student from their local community.
