Initially, it is important to assess the situation to see why Mrs. Smith
needed the Amoxicillin, and why she is requesting a refill. Whatever she
is presenting with could result in her needing another type of
antibiotic. As the nurse practitioner in the situation, it is important
to obtain all information possible as to how the antibiotic was
provided. The next step needed would be to speak with Stephanie, and
obtain her side of the story. If indeed, it would then be appropriate to
take this situation to your supervisor. It is important to obtain
everyone’s story initially, and then go about the proper chain of
command.

One major concern regarding this situation is the interactions
and side affects this medication could have caused. Without seeing Mrs.
Smith for one year her health history could have changed. Antibiotics
can interfere with other medications such as Coumadin, Lithium,
etc.…Drug hypersensitivity is an important consideration, and a common
strategy to avoid giving patients medications when a high likelihood of
reactions exist (Legendre, Muzny, Marshall, & Swiatlo, 2013). This
situation could have gone south and caused harm to the patient, and
could have placed the advanced nurse practitioner’s license at risk. It
is important to know your role when working in the medical field. Any
small change or alteration could be the cause between life or death.
Advanced nurse practitioners have ethical responsibilities not only to
their employer, but also to their patients (Buppert, 2015).
