CHAPTER 4 Case Studies Healthcare and Nursing
CHAPTER 4 Case Studies Healthcare and Nusing
Real-World Case
According to the American Academy of Family Physicians (AAFP), “Continuity of care is the process by which the patient and the physician are cooperatively involved in ongoing health care management toward the goal of high quality, cost-effective medical care” (AAFP 2010). The SAM (Formerly CCR), an XML-based health data exchange standard, was created to help communicate summary health information from one provider to another for referral, transfer, or discharge of the patient. Microsoft introduced a personal health record platform that allows sharing of health information.

Microsoft’s HealthVault personal health records (PHR) allows the user to gather, store, and share health information online. HealthVault supports the CCR and CCD standards for exchanging patient data. Not only can applications use the CCR standard to exchange data with HealthVault, but a user can extract and reconcile data from a CCR (Microsoft 2009). Microsoft’s partners include New York-Presbyterian Hospital, Health Partners, Kaiser Permanente, Aetna, Walgreens, among others.
Real-World Case Discussion Questions
1. With companies like Microsoft entering the PHR field, what concerns result?
2. Why are standards such as the CCR important to healthcare?
Application Exercises
Instructions: For each item, complete the statement correctly or choose the most appropriate answer.
1. Identify four data elements about yourself or another individual and convert them into information as shown in the following example:
Data elements:
Age: 32
Gender: Female

2. Pretend Natasha Fatale is a nursing home resident at the time of admission, and conduct an interview on the basis of the Minimum Data Set for Long-Term Care. Use the data collected in the MDS to complete a resident assessment protocol summary, and identify at least two RAP triggers. The current version of the MDS may be found at http://www.cms.hhs.gov/NursingHomeQualityInits/20_NHQIMDS20.asp#TopOfPage
Use the menu on the left side of the home page to select MDS 3.0 for the current version of the MDS.
3. Use the AHIMA library to access one article about an HEDIS outcomes study and one article about a core measure study using ORYX indicators. Summarize each of the articles in a one-page review.
4. Develop a table or grid that identifies relevant healthcare informatics standards that should be used to design an electronic health record (EHR) in an acute care setting. The EHR should include data from the following systems, and the data will be used for claims processing/billing as well as for patient care:

• Clinical data from caregivers (for example, history and physical exam, nurse’s notes, physician’s progress notes, physician’s orders, consultations, final diagnosis)
• Pharmacy
• Laboratory
• Radiology
• Emergency/trauma center (for patients admitted through the emergency department)
• Coded data for statistics, research, and billing
5. Write a memo to the chief executive officer (CEO) of a data committee to oversee implementation of healthcare informatics standards. In no more than one and a half pages, persuade the CEO that the committee is needed
