Patient:

Alex, a64-year-old man, is married with4 children and 1 grandchild.

Past Medical History:

He has a long standing history of emphysema, known COPD. Despite his
history of COPD,Alex struggled to give up smoking.
He weighs 101kg and his BMI is above 31. He enjoys eating fast food and
do not see the need for healthy eating.
Alex is isolated and at times withdrawn due to having lost his job
recently. He finds comfort in smoking 20 cigarette a day and does not
see the point of stopping.

Mum: COPD, Nil other.

Current medication:
Once-daily long-acting muscarinic antagonist and, long-acting beta2
agonist with inhaled corticosteroid in a combination inhaler.
He does not take recreational drugs, or over the counter medications/
herbal remedies.
Alex has refused an annual influenza vaccination.

Present situation:
Alexwas seen by the intermediate care team at home complaining of
worsening breathing difficulties over the last few days. When the nurse
arrived, he was sitting in his armchair with his home oxygen turned on
(2L via naso-specs).
He has had an upper respiratory infection for about 2 weeks. Alex would
prefer to stay at home rather than get admitted to hospital. However,
the nurse has encouraged him to be admitted to your ward for further
assessment.

His initial observations are:

SpO2: 86% breath sounds = wheezing, pursed lip breathing, using
accessory muscles when breathing, peripheral signs of cyanosis
